5/30/01 -- 2009 PHYSICAL INFORMATION
In compliance with PIAA regulations, the sports physical process has changed, effective for the 2009-10 school year. Please read the following instructions carefully:

On JUNE 12, 2009, physicals for fall sports will be given from 1:00 - 5:00 pm with registration being in the high school gym. July 10, 2009 is the tentative date for make-up physicals.

Every student participating in any interscholastic sport will be required to use the PIAA Comprehensive Initial Pre-Participation Physical Evaluation (CIPPE) form. The CIPPE form must be preformed no earlier than June 1, 2009 and will be effective until May 31, 2010. If your child plans on participating in any interscholastic sport for fall, winter, and/or spring, the CIPPE form can be completed any time beginning on June 1, 2009.

Parents/guardians must complete and sign Sections 1, 2, and 3 of the CIPPE form for initial certifiation (for the first sport in the school year). An Authorized Medical Examiner must complete Section 4. You have two options to have this completed:

(1) Your child's primary care provider may complete the form or

(2) Physicals will be performed at the high school for each season by Geisinger's Sports Medicine Department for a fee of $5.00 and will be collected at the time of the physical. Make checks payable to: Danville Area School District.

Be aware that 11th graders need several physicals throughout the course of their junior year. If your child's primary care provider will be completing the CIPPE form for sports, the working and driving permit physicals, and the mandated school physical can be completed at the same time. Please remember to take all forms with you to your doctor's office. Please understand that the sports physicals held at the school are for sports only. DO NOT bring mandated school physicals, working permit or driving permit physical forms for the doctor to sign at sports physicals.

CIPPE forms will be available at the sign-up meeting on 6/2/09 and are available in the offices at the High School and Middle School and on the district website.
